Orgasm After Prostate Cancer Treatment (Video)
What orgasm is, treatment-related changes, and ways to improve or recover orgasm
This video covers:
- What is orgasm: Considered the height of sexual excitement; men can be orgasmic without an erection, and orgasm isn’t required to experience sexual pleasure.
- How treatments can affect orgasm:
- Active surveillance: Possible changes if anxiety is present.
- Prostatectomy: No ejaculation (“dry orgasm”); possible climacturia.
- Radiation: Blood-tinged ejaculate; decreased ejaculate volume.
- Androgen deprivation therapy: Challenges with reaching orgasm.
- Steps to improve/recover: Stay present; review medications; consider blood hormone monitoring; use manual/oral stimulation (orgasm can occur without erections); adjust breathing (faster may hasten, slow deep breathing may prolong time); perform Kegel exercises; use good sexual lubricant.
- Optimize stimulation: Some respond best to continuous stimulation; others to start-stop patterns that build sexual tension.
- Try different tools & approaches: Vibrators may intensify stimulation of orgasm nerves (and may also stimulate the cavernous nerves); incorporate psychological/mindfulness-based arousal techniques (e.g., tantric or mindful approaches).
- Key reminder: The prostate cancer experience can be an opportunity to learn new ways of being sexual, including new experiences with orgasm.
